- Monitor and document changes in patients conditions and promptly report any concerns to the appropriate healthcare professionals
- Care Planning: Collaborate with the healthcare team to develop individualized care plans for patients, considering their medical conditions, preferences, and resources
- Ensure that care plans are regularly reviewed, updated, and implemented effectively
- Medication Administration: Administer medications following established protocols and guidelines
- Ensure accurate dosage, proper administration techniques, and appropriate documentation of medication administration
- Patient Support and Education: Provide emotional support and educate patients and their families about their conditions, treatments, and self-care strategies
- Answer their questions and address their concerns, promoting a positive patient experience
- Wound Care: Perform wound assessments, dressings, and other related procedures as required
- Monitor wound healing progress and collaborate with the healthcare team to implement appropriate interventions
